This affidavit is required when authorities, institutions, or various entities need official proof of a residential address — for embassy proceedings, foreign tax authorities, academic institutions, or bureaucratic agencies.

What is a residency affidavit?

A residency affidavit is a sworn statement made before a licensed notary under the Notaries Law, 1976 (חוק הנוטריונים, תשל״ו-1976). The declarant confirms their permanent residential address, duration of residence, and other details as needed. The notary verifies identity, explains the legal meaning, and certifies the signature with an official notarial seal.
